Content on this site is general information, not medical or legal advice, and does not create a doctor–patient relationship. It is not a substitute for professional care. For a medical emergency, call 911.

No service yet

We are not currently issuing doctor's notes, providing medical care, or taking payment. Reserving a note records your interest so we can contact you at launch — no clinician reviews your reservation, and it does not create a doctor–patient relationship or guarantee future service, pricing, or coverage in your state.

When the service launches

At launch, notes will be issued only after a review by a physician licensed in your state, who may decline to issue a note in their professional judgment. CuroNote does not provide diagnosis, treatment, prescriptions, controlled substances, FMLA, disability, or fitness-for-duty documentation. Full service terms will be presented before you purchase anything.

Acceptable use

You agree not to misuse the site, submit false information, or use any content to create fraudulent documents.

Intellectual property

All content on this site is owned by CuroNote or its licensors and may not be copied or republished without permission.

Disclaimers & limitation of liability

The site is provided "as is," without warranties of any kind. To the maximum extent permitted by law, CuroNote is not liable for any indirect or consequential damages arising from your use of the site.
